Schedule & Compensation:

• Position: Full-Time (40 hours/week)

• Hours: 8:00 AM – 4:30 PM (flexible based on individual needs)

• Starting Salary: $55,000 annually (commensurate with experience)

Job Description

• Develops and maintains professional relationships with staff, physicians, residents, families, community resources, and third-party payers.
• Completes Admission forms/assessments and documentation as outlined in the Social Service Admission Process.
• Completes a plan of care for each resident based on their needs. Documents on progress towards care plan goal, and updates POC with significant change and/or quarterly.
• Assess residents various health needs on an on-going basis. Assess the need for referrals to health services and provides education and support.
